Requirements of immunizations/CPR (Updated 2-18-2021) 

 

TB (tuberculosis) Screening *YEARLY REQUIREMENT

There must be documentation of one of the following:

  • Proof of skin tests (2 separate skin tests placed no sooner than 7 days apart and no longer than 9 weeks apart. This will be a total of 4 steps; two tests and two reads.
  • One (1) Negative QuantiFERON Gold Blood Test (lab report required).
  • T-SPOT TB test
  • If the results are positive, a clear Chest X-Ray (with lab report) is required. If you have documentation of INH Prophylaxis treatment, submit this as well.
  • Students that are renewing their TB requirements PRIOR to expiration date of original immunization only need 1 skin test (if they previously received two skin tests) or a QuantiFERON Gold blood test (if they previously received a blood test) to be compliant. 
  • If annual TB expired, you MUST complete the 2 skin tests or QuantiFERON Gold blood test again for compliance.

 

Tetanus, Diphtheria & Pertussis (Tdap) *GOOD FOR 10 YEARS

There must be documentation of the following:

  • Tdap immunization within the past 10 years. 
  • Must be a Tdap NOT a TD or Dtap immunization.

 

Varicella (Chicken Pox) *LIFETIME COVERAGE (both in series must be done before being able to register)

There must be documentation of one of the following:

  • 2 vaccinations given at least 4 weeks apart.
  • Positive antibody titer (lab report required)
  • If the titer does not show immunity, then 2 varicella immunizations must be repeated

 

Measles (Rubeola), Mumps & Rubella (MMR) *LIFETIME COVERAGE (both in series must be done before being able to register)

There must be documentation of one of the following:

  • 2 vaccinations given at least 4 weeks apart. 
  • Positive antibody titers to Measles, Mumps, and Rubella (proof of immunity through a blood test for all 3 components).
  • If any component of the MMR is negative, the MMR (2 vaccinations) must be repeated.

 

Hepatitis B *LIFETIME COVERAGE (1&2 in series must be done before being able to register)

There must be documentation of the following: (Immunizations OR antibody titer)

  • Documentation of immunizations required (all 3 immunizations--1st immunization, 30 days later 2nd immunization, 3rd immunization must be a minimum of 5 months after the 2nd dose) 
  • 2 dose Heplisav-B is also accepted
  • Documentation of a positive (reactive) antibody titer (lab report required).
  • If non-reactive titer is found, a new series of 3 immunizations must be completed; (1st immunization, 30 days later 2nd immunization, 3rd immunization must be a minimum of 5 months after the 2nd dose).

 

CPR Certification *MUST BE CURRENT

  • Certification MUST be through American Heart Association Basic Life Support for the Healthcare provider.  
  • American Heart Association (AHA) go to www.heart.org for class information. No instructor cards will be accepted. CPR certifications usually need to be renewed every 2 years.
  • There are CPR classes available on campus that you can complete for your certification. Dates and times will change semester to semester. Please reach out to Joell Goff at jgoff@ameritech.edu to find out the available CPR classes.

Flu Vaccination 

Flu shot must be for the current flu season August through May season. 

  • Serum given must be for the current season- encouraged to get flu shot no earlier than August of the year.
  • No receipts will be accepted. There must be documentation with signature of the individual who administered the immunization.  

 

 

To be accepted all documents must show student name, date immunization was administered, and facility name or be on official letterhead/document.
